When you contact Prima to fill your Lupron Depot prescription, let us know you have the co-pay savings card. You can email a photo of it, text it to (718) 230-3535, or mention it when you call. The savings card has an expiration date printed on it. Most cards are valid for the calendar year in which they are issued. You will need to re-enroll at the start of each new year. Enrollment takes the same four steps and is quick once you know the process.
